After Headswap FlexArray is unable to see drives from E-Series Backend

Issue

After replacing an HBA used for back-end array LUNs access in a fabric-attach configuration, ONTAP may show continuous hang condition and will not have access to the back-end E-Series array(s). This issue doen not apply to a direct-attached configuration. 

The following errors will be printed to ONTAP console:

Jun 07 13:39:03 [ClusterA-01:fci.adapter.error:notice]: Fibre Channel adapter driver encountered error "Device asyncd state stalled" on adapter 2b. 
Jun 07 13:39:04 [ClusterA-01:fci.adapter.error:notice]: Fibre Channel adapter driver encountered error "Device asyncd state stalled" on adapter 2a. 
Jun 07 13:39:08 [ClusterA-01:fci.nserr.noDevices:error]: The Fibre Channel fabric attached to adapter 2a reports the presence of no Fibre Channel devices. 
Jun 07 13:39:08 [ClusterA-01:fci.nserr.noDevices:error]: The Fibre Channel fabric attached to adapter 2b reports the presence of no Fibre Channel devices. 
Jun 07 13:39:13 [ClusterA-01:fci.adapter.error:notice]: Fibre Channel adapter driver encountered error "Device asyncd state stalled" on adapter 2b. 
Jun 07 13:39:14 [ClusterA-01:fci.adapter.error:notice]: Fibre Channel adapter driver encountered error "Device asyncd state stalled" on adapter 2a.

When looking at the DQ traces of the E-Series controllers at the time, every FC login attempt is reporting logout of the old initiator and reports duplicate port ID:
