All paths down reported on RHEL host and host lost access to storage
Identify and eliminate any causes that might trigger the APD state of the storage device:

Possible fabric issues.
Verify end to end connectivity
Verify all end port states during the issue time.
Verify lun state on storage end when the issue occured.
