Switchback failed: Error on "receiver" stream

Issue

During a head upgrade in an MetroCluster setup, a Switchback might fail with the following errors: 

::*> metrocluster operation show

       Operation: switchback
           State: failed
      Start Time: 8/20/2021 11:49:47
        End Time: 8/20/2021 11:59:42
          Errors: Failed to set up the node and cluster components for the switchback operation.
                  ClusterA (fatal): A stream operation timed out.
                  Error: Failed to sync configuration on cluster "ClusterB". Error on "receiver" stream: (Receiver Thread  
                  failed to process baseline message on streamID <stream-id>. 
                  Error: duplicate entry).
