FC-VI adapter: Hardware initialization failed for port

Issue

Following an FCVI reset the FCVI ports fail to initialize and remain offline:
[ClusterA-01: ispfcvi2500_main1: fcvi.qlgc.system.error:error]: FC-VI adapter: System Error on port 0e. Resetting the port.
[ClusterA-01: ispfcvi2500_main2: fcvi.qlgc.system.error:error]: FC-VI adapter: System Error on port 0f. Resetting the port.
[ClusterA-01: ispfcvi2500_port2: fcvi.update.link.state:notice]: FC-VI adapter: Physical link is down on port 0f.

Adapter does not recover after a reboot and during boot reports an initialization failure:
