FC-VI adapter: Hardware initialization failed for port

Issue

  • Following an FCVI reset the FCVI ports fail to initialize and remain offline:
[ClusterA-01: ispfcvi2500_main1: fcvi.qlgc.system.error:error]: FC-VI adapter: System Error on port 0e. Resetting the port
[ClusterA-01: ispfcvi2500_main2: fcvi.qlgc.system.error:error]: FC-VI adapter: System Error on port 0f. Resetting the port
[ClusterA-01: ispfcvi2500_port2: fcvi.update.link.state:notice]: FC-VI adapter: Physical link is down on port 0f
 
  • Sysconfig -A output shows the physical link as down:
slot 0: FCVI Host Adapter 0e (QLogic 8324(2672) rev. 2, unknown)
